B. Instructions for Submitting Application Form
- Read all instructions and download the current prescribed application form (Word format).
NOTE: If you have trouble downloading the application form, clear your internet browser cache (“clear browsing data”) and try again.
- Answer every question on the application form that applies to you. If a question does not apply to you, type “N/A” in the space provided.
Use Arial font, 11 point, black.
DO NOT highlight or otherwise embellish the application.
DO NOT use ALL CAPITALS.
Follow the word count restrictions, where applicable. To find out your word count, highlight the text with your cursor, and look at the status bar at the bottom left corner of your window, or highlight the text with your cursor and click on the “Review” tab in the set of tools at the top (ribbon area), then click on “Word Count”.
Do not rely on AI tools to answer any of the questions. You are encouraged to check for correct spelling and grammar. Your responses should be original and will be evaluated as part of the application process.
- Scroll to the bottom to review important information about completing the application form.
- Save your application as a PDF document.
Review your completed application form carefully. Save the document as a PDF file with the following file naming convention (note use of uppercase and lowercase letters):
LASTNAME_FirstnameMiddleInitials – Region(s) applied
File naming tips:
- Type your last name in all uppercase letters with no spaces.
- Type hyphenated names as well as two first names or two last names as one word.
- Do not use apostrophes.
Examples of application file names for those who are applying to vacancies in one region only:
- Carlo Anthony J. D’Agostino is applying to Ottawa and Ottawa (Indigenous) in the East Region:
DAGOSTINO_CarloAJ – East.pdf
- Robert John de los Reyes is applying to Brampton and Hamilton in the Central West Region:
DELOSREYES_RobertJ – CW.pdf
Examples of application file names for those who are applying to vacancies in more than one region:
- Sue-Ellen Barbara MacAfee is applying to Newmarket, Hamilton and Toronto in the Central East, Central West and Toronto Regions, respectively:
MACAFEE_SueEllenB – CE, CW, Toronto.pdf
- Catherine Ellen A. O’Connor is applying to St. Catharines (Indigenous) in the Central West Region and Kitchener (Indigenous) and Owen Sound (Indigenous) in the West Region:
OCONNOR_CatherineEA – CW, West.pdf

- Use the following e-mail subject line:
The subject line of your e-mail will be the same as the file name of your PDF application form without the “.pdf” extension.
Subject: LASTNAME_FirstnameMiddleInitials – Region(s) applied
Using the examples in #5 above, below are the corresponding e-mail subject lines:
- Subject: DAGOSTINO_CarloAJ – East
- Subject: DELOSREYES_RobertJ – CW
- Subject: MACAFEE_SueEllenB – CE, CW, Toronto
- Subject: OCONNOR_CatherineEA – CW, West
- E-mail your completed application to JPAAC.Application@ontario.ca.
You are required to complete and submit only one justice of the peace appointment application form even if you are applying to more than one vacancy or more than one region.
Do NOT include a message to JPAAC in the body of your e-mail as it will not be read and answered. Your e-mail may include a standard signature that includes your contact information.
- Deadline: All completed applications must be received by JPAAC by 11:59 p.m. EST on Sunday, November 9, 2025. Late applications will not be considered.
After you submit your application to JPAAC.Application@ontario.ca, you will receive an automated e-mail acknowledgement stating that your application has been received by JPAAC.
